Shows name, age, gender, marital status, date of birth and place of birth of patient; date admitted; prior address; condition on admission; and date of discharge or death.  The Infirmary register prior to 1867 has been lost as reported in the 1883 History of Union County, Ohio .  Certain portions of the records are restricted pursuant to ORC 149.43 due to the fact they contain medical information.
